Information for Students
**Network access expires every 12 months. If you receive an error when using your OSU credentials (WITHOUT "@osu.edu" in the Username field), your supervisor or dept admin will need to submit a CVM Access Request on your behalf - users cannot submit access requests for themselves. You must also be compliant with OSU's annual Institutional Data Policy training before accessing this form. You may contact CVM IT directly at go.osu.edu/vis if you are still unable to log in after completing these first two steps.**
Each year, graduate students in the College of Veterinary Medicine are required to complete an Annual Progress Report. This form serves as a summary of your accomplishments and progress in your graduate study over the course of the last calendar year. Please include information from January 1, 2023 to December 31, 2023. This report will be reviewed by your advisor, your committee, and the Council for Graduate Studies to ensure that you are making satisfactory progress toward your degree and to have a record of your accomplishments over the last year.

It is highly recommended that you download this document and prepare all of the necessary information BEFORE you begin entering it into the electronic form. If information is not properly formatted, your report will be returned to you to resolve these issues before it can be forwarded to your advisor.
All research plans must be submitted using this template: Download Research plan template Any plan submitted in another format will be deemed unacceptable and your report will be returned to you for revision.
If necessary, you can save the form and return to it later. A saved form is not submitted until you complete all of the required fields and hit the “Submit” button.
